Athletic physicals for next school year

Physicals are required for all Calhoun County ISD athletes, band members, cheerleaders, drill team, ROTC cadets, etc. and will be administered on May 2 (for male athletes) and May 3 (for female athletes) for $20 (cash only). This is a one time event, coordinated by our CCISD athletic trainers; however, if your family prefers to have a physical completed by your personal doctor, that is fine, but please use the UIL form:

May 2 - Physicals for male athletes, 5:30 - 7 p.m. at the CHS Game Gym hallway.
May 3 - Physicals for female athletes, 5:30 - 7 p.m. at the CHS Game Gym hallway.
* No check-in will be allowed after 7 p.m.
* Cost is $20 - cash only
In addition to the medical physical, there is also an online form that parents need to submit. Here are the instructions for the online form:
1) Go to https://www.rankone.com/content/ and click on “Parents Click Here”
2) Click on Online Forms and select Texas as the state. Then find “Calhoun County ISD”
3) Afterwards, the Parent Portal will appear. Click on “Proceed to Online Forms”.
4) Click on “Continue as Guest”. DO NOT MAKE AN ACCOUNT.
5) Click on UIL Forms Signature Page and Emergency card and fill and sign each blank that is required.
